Support Team

A dedicated team of social workers helping Perinton seniors connect with services needed to continue to live independently or find the right placement for care.  The support team is responsible for care management, transportation, assessment, benefits support, crisis intervention, counseling, family support, home upkeep, friendly visits, health education, meal delivery, falls prevention, and so much more.

Contributions; PCH, Town and Village, individuals, groups.
Over $100,000 from Crafts Bits and Pieces volunteer-staffed store in Fairport Village Landing.
